New Patients

The doctors welcome new patients who live within our practice area which is Penrith and surrounding villages. We will need your medical card or your NHS number. Medical treatment is available from the date of registration.

If you live within the green inner boundary we may be able to register you as a new patient. The red outer area is where if existing patients move to they can remain registered with the practice.

Patient Registration

If you wish to register online please click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. Your registration is only complete once you have signed the form and been accepted by the practice.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice. Or you can pick up a form from the surgery.

You will also need to bring in 2 forms of identification with you, 1 will need to be photographic (eg UK driving licence or passport) and a utility bill/statement that has been sent in the last 3 months.

Medical questionnaire
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
